A minimum of level 5 Qualification in Healthcare, Childcare or any relevant course. 39hours/weekProvide support to service user in a placement/residential setting.Liaise with other multidisciplinary team.Support Service user with meaningful and relevant daily activities.Planning, implementation and evaluation of service user care plan.Required to send daily report to the team leader.
